Vaccine policy
The providers at University Pediatric Associates believe strongly in protecting children from vaccine-preventable diseases. We follow the guidelines of the Centers for Disease Control (CDC) and the American Academy of Pediatrics (AAP) and require that all patients are fully immunized following the recommended schedule of immunizations. Appointment policies
Although we understand that unexpected circumstances may arise, we ask that all families abide by the following expectations.
- Late Arrivals: If you are late for a scheduled appointment, it may be necessary to reschedule your appointment.
- Cancellations: We require 24-hour notice for cancellation of appointments. This is so that we may schedule another patient in the cancelled appointment time. Failure to comply with the 24-hour notice may result in a $25 cancellation fee.
- Missed Appointments: After three missed appointments without a cancellation call, we reserve the right to terminate the physician/patient relationship.
