Vaccine policy

The providers at University Pediatric Associates believe strongly in protecting children from vaccine-preventable diseases. We follow the guidelines of the Centers for Disease Control (CDC) and the American Academy of Pediatrics (AAP) and require that all patients are fully immunized following the recommended schedule of immunizations. Appointment policies

Although we understand that unexpected circumstances may arise, we ask that all families abide by the following expectations: If an appointment is cancelled with less than 24 hours’ notice or if a patient fails to arrive for an appointment, a $25 fee will apply. Missed appointments without a cancellation call may result in termination of the physician/patient relationship. Arriving to an appointment 15 or more minutes after the appointment time will result in a missed appointment, and a $25 fee will apply.
